The Grey Bruce Community Health Corporation is a non-profit charitable organization. It was established in 1985 to develop a number of community mental health and addiction programs across Grey-Bruce. The Corporation currently operates 2 addiction programs and 6 community mental health programs, and sponsors 3 Community Investment Fund (C.I.F.) Projects. Our major funder is the Ministry of Health and Long Term Care. MISSION OUR MISSION IS HEALTHIER INDIVIDUALS, FAMILIES AND COMMUNITIES IN GREY-BRUCE. IN ORDER TO ACHIEVE OUR MISSION, WE WILL: 1. Develop and operate high quality community-based programs and services. 2. Establish effective partnerships with our stakeholders. 3. Promote service coordination and continuity of care at the client, program and system levels. 4. Promote mental health and prevent mental illness and addiction. 5. Make effective use of our resources. BOARD OF DIRECTORS The Corporation is directed by a 10-member volunteer Board of Directors who are residents of Grey and Bruce Counties. A COMMITMENT TO PARTNERSHIPS The Corporation is a member of Mental Health Grey-Bruce, a partnership of four local organizations that provide mental health services. The partnership was established in January, 1999 to enable the four organizations to develop and operate multi-agency Community Mental Health Teams across Grey-Bruce. Each Team includes staff of the partner organizations who, with supervision of a Team Leader, provide a range of mental health services to a designated catchment area. The goal of Mental Health, Grey-Bruce and of its Community Mental Health Teams is to enable people with serious mental illness and/or acute mental health needs and their families to access effective, coordinated mental health services as close to home as possible. OUR PROGRAMS Mental Health Programs Community Network Support Team Provides community development services to enhance mental health knowledge and resources in Grey-Bruce, and promote effective communication and coordination amongst system stakeholders. Mental Health Evaluation Group An interagency group that operates a computerized information system used by 19 local hospital and community mental health and addiction programs. The goal of the system is to facilitate mental health and addiction planning at the program and system level. Bruce and Grey Primary Mental Health Counselling Services Provide mental health counselling for adults with serious, ongoing and/or acute mental health problems and their families, as well as post-emergency counselling for women survivors of sexual assault or childhood sexual abuse or incest. These programs host 2 of the multi-agency Community Mental Health Teams in Grey-Bruce. Community Connections: Housing & Support Provides housing and support for adults with serious mental illness. Housing services include: affordable, supported accommodation for up to 16 adults in 2 residential settings (Brooke House, an 8-bed group home, and Riverview Apartments, a 6-unit apartment building) and rent subsidies to enable up to 49 individuals who are homeless or at risk of homelessness to obtain affordable apartment accommodation in the community. Support services include one-to-one outreach support, individual and group life skills training and social/recreational activities. Also provides housing supports, including a Housing Registry. Family Support Initiative Provides education, consultation, and community development services to foster networking and peer support amongst family members of those with serious mental illness across Grey-Bruce. Consumer/Survivor Development Project Provides education, consultation and community development services to foster self-help, networking and leadership development amongst consumer/ survivors with serious mental illness. Addiction Programs New Directions for Alcohol, Drug & Gambling Problems Provides community-based addiction services for adults with substance abuse and/or gambling problems and those who are affected by someone else's substance abuse and/or gambling problems. Services include: comprehensive assessment; treatment planning and referral; outpatient counselling; case management and follow-up. The program operates a full time office in Owen Sound and provides regular outreach to 7 other communities across Grey and Bruce (Meaford, Durham, Hanover, Walkerton, Kincardine, Wiarton and Southampton). CHOICES: Drug & Alcohol Counselling for Youth Provides community-based addiction services for youth with substance abuse problems, those who are affected by someone else's substance abuse, and those who are at risk. Services include: comprehensive assessment; treatment planning and referral; outpatient counselling for youth and family members; case management and follow-up. Services are targeted to youth aged 16 - 20 years, however the program also services a significant number of youth under 16 years. The program operates a full time office in Owen Sound and provides regular outreach to 9 high schools in 8 other communities across Grey-Bruce.(Flesherton, Walkerton, Hanover, Kincardine, Port Elgin, Wiarton, Meaford and Chesley).
