About the program:
A dual diagnosis refers to people who have a developmental disability and a mental health problem (such as a serious mental illness, aggression, self-injury). A Coordinator, an RPN Dual Diagnosis Case Manager, and a consulting psychiatrist provide Grey and Bruce counties with assessment, consultation, treatment and support to health care professionals, developmental service providers, individuals and families living with a dual diagnosis. This program is based from Grey Bruce Health Services.
Services provided: psychiatric consultation for developmentally disabled people ages 16+, with recommendations for family physicians psychiatric followup for people with ongoing needs assessment and monitoring of client response to treatment, in the community planning and evaluation of treatment strategies in collaboration with client, family and developmental service providers coordination of community services as early as possible consultation, support and communication with person's support system if they have been admitted to hospital locally or in London information about local and regional educational, clinical and service resources for this population assisting with planning for local and regional changes to resources for this population Can we help you?
