Who are we?
The Community Outreach Treatment Team (COTT) is a team of mental health professionals that provide support services, both in our office and in the community, to people with acute mental health problems. We are committed to:
providing flexible and individualized services for clients and families in their home community improving the coping skills of the individual, family and others in the support system building partnerships with other mental health providers in Grey and Bruce counties What do we do?
COTT provides intensive mental health services to clients in the community with the goal of stabilizing the client's condition and making community referrals. The treatment is similar to that offered in hospital except that it is provided in an office and the client's home. Clients are visited by a team member who assesses the problem with the client. A treatment plan is developed jointly by the COTT staff, the client and his/her family. The plan includes such things as individual counselling or therapy, couple and family support, educational and/or support groups, and medication information. When required, referrals to other community services are made prior to discharge. Whom do we serve?
Referrals for the COTT team must come from a psychiatrist, or from a staff member of the Inpatient Psychiatry Unit or the Crisis Intervention Team. Those referred for service: must be at least 16 years of age must live in Grey or Bruce counties must not have alcohol or drug dependencies as their primary problem must be willing to receive help in their own home or at the program office should have a problem which requires urgent and intensive services Is there a cost?
COTT is a program of the Grey Bruce Health Services and is funded by the Ministry of Health and Long Term Care. Our services are offered free of charge. Can we help you?
