Who are we?
The Assertive Community Treatment Team is a team of mental health professionals who provide services aimed at treatment, rehabilitation and support for people living in the community with serious mental illness.
Whom do we serve?
We provide services to adults (16 years and over) living in Grey or Bruce counties who have a serious mental illness. Our services are geared to those people who have complex needs related to severe and persistent mental illness and who are frequent users of mental health and community services. Referrals to the ACT Team can be made by clients, family members, physicians and community agencies.
What are our key services?
Key services provided are: direct assistance (food, shelter, self care and finances) medication services skills training vocational and recreational services supportive services family education and support
What are some of the characteristics of our services?
Characteristics of our service model include: an assertive outreach community based services provided seven days a week clients can receive long-term service we develop individualized treatment plans there is a shared responsibility for treatment among team members there is an integration of treatment, rehabilitation and support
What are the hours that services are available?
Services are available seven days a week, 365 days a year. Staff are available from 8 a.m. to 9 p.m. Monday through Friday and from 8 a.m. to 8 p.m. on weekends. During afterhours the Crisis Team at the Grey Bruce Health Services provides coverage for our clients.
Is there a cost?
The ACT Team is a program of the Grey Bruce Health Services and is funded by the Ministry of Health and Long Term Care. Our services are offered free of charge.
